Important Forms
The followings forms will need to be signed acknowledging your agreement before you can begin to service clients as an Independent Contractor with Trendsetter's W@H. Please review them carefully as the forms define and describe your rights as an Independent Contractor, the rights and limits of liability for Trendsetter's W@H and Arise™ Virtual Solutions. In additional, Trendsetter's W@H signs a Statement of Work (SOW) upon the completion of the CSP's (Customer Service Professional) certification that outlines your responsibilities as a CSP and includes but is not limited to the following provisions: 1) a description of services; 2) certification requirements; 3) service intervals; 4) service requirements; 5) consequences of fraud and unprofessional business practices; 6) Arise™ support services; 7) service revenue; 8) representations and warranties; 9) terms and terminations; 10) the company's policy on the use and protection of personally identifiable information and other important policies and requirements. Copies of these signed documents are also sent to the CSP.
When you have completed certification, the CSP and a representative of Trendsetter's W@H will schedule a meeting to discuss these documents.

The forms represent an agreement between Trendsetter's W@H and Arise™ reaffirming that the CSP is not an employee of Arise™ but is an employee of Trendsetter's W@H and is not entitled to any Arise™ employee benefits. As an Independent Contractor working with Trendsetter's W@H, you are paid by Trendsetter's W@H and any benefits derived from your association with Arise™ will come from Trendsetter's W@H.

Taxpayer Identification Certification W-9 Form
Trendsetter's W@H does not deduct federal, state, social security, medicare, unemployment compensation, or workman's compensation from the Independent Contractor's compensation. The W-9 form is required to produce a yearly 1099-Misc at the end of the year for tax purposes. The Independent Contractor is responsible for paying any taxes or other benefits directly to the Federal or State government. Information on Self-Employment Taxes (Social Security and Medicare) and Income Taxes for self-employed individuals can be found on the IRS website. Form 1040es is the IRS form for self-employed to calculate the Estimated Tax for Individuals. Please visit the IRS website for information on completing and submitting this form.

Payroll Processing Agreement
Your compensation will be processed and deposited or mailed on the 15th and the last day of the month. The CSP has an option to have their compensation directly deposited into their bank account, applied to a Bank of America CashPay prepaid Visa card, or mailed. This will be set-up prior to your first pay period so it is important to return the signed forms to Trendsetter's in a timely manner. Without these forms on file, your check will be mailed to the address on file. You have the option to electronically sign the documents, email the documents to d.owens@trendsetterswah.com or mail them to Trendsetter's W@H Customer Care, 4012 S Rainbow Blvd, Ste. K-564, Las Vegas, NV 89103. If you choose to electronically sign, please call or send an email so that the proper forms can be sent to your email address.
